Why was I chosen?
To help the Oregon Health Authority understand the experiences of many different people who they help, many different individuals have been chosen to represent others like them across the state. If you’ve received a letter in the mail, a phone call, or a survey booklet, then your voice is the one we’ve been looking for. Your answers will tell about not just your situation, but of those like you throughout Oregon.
This survey is sent to individuals who receive outpatient services, services in a residential setting, the family members of youth who receive children and young adults themselves who receive services. We are looking for a wide ranging, diverse group of voices to tell us about the mental health services they’ve received since January 1st, 2023

What is this survey for?
Every year, the Oregon Health Authority (OHA) partners with independent researchers to conduct a survey of individuals who receive mental health services through OHA supported programs. This survey helps OHA design initiatives to improve these services and provide people the chance to be frank about the quality of care they receive from their providers.
Are my answers confidential?
Your identity and your answers are never presented on their own. They will be combined with others and presented in aggregate. Your confidentiality is of the utmost importance in this research.
When will I receive my gift card?
Gift cards are sent to respondents via email through our partner “Tremendous” usually within ten business days after you complete your survey. Look for an email with the subject line “OHA and MDR sent you a reward – $10” from “OHA MHSIP Survey”. If you haven’t received the digital gift card, or have any questions, please reach out and contact us.
